Your Isabella Nurse Care Manager provides...

Once you are a member, an Isabella Nurse Care Manager will develop your personal care plan and provide consistent attention, month after month.

• Behavioral health oversight to monitor you for depression or other emotional and psychological problems which may need attention.
• Social care services involvement through Isabella’s own staff, helping you with referrals, program entitlements, financial management and other vital matters.
• Community involvement and participation. Your Isabella Nurse Care Manager knows your community, and can help you network and get additional local services.
• Medical care coordination to guide you in your communication with your doctor and to assist you in understanding and meeting your medical needs.

Consistent, reliable help in your home

As an Isabella Home Care participant, your daily care is provided by an Aide closely monitored by an Isabella Nursing Care Manager who visits you at least once every two weeks.

Aides follow a unique, personally-developed care plan developed in conjunction with your needs and preferences and with your physician's authorization. It’s a close, personal, yet professional relationship.

In addition, you’ll receive the practical day-to-day help you need to keep living safely at home. Your Isabella aide can help you prepare meals, bathe and dress, do light housekeeping, accompany you out of your home for shopping or medical appointments in your neighborhood. We even help you arrange for services such as heavy housekeeping, home maintenance and repairs.

Isabella’s Home Care program is open to residents of Manhattan and The Bronx age 18 and older who are under a doctor’s care and have a chronic medical or mental illness or disability and need periodic skilled nursing.
