A performance improvement manual

Isabella Geriatric Center and Cobble Hill Health Center are pleased to announce the publication of an informative and timely new manual for Nursing Home Staff.

Getting Better All The Time

Working Together for Continuous Improvement

A Guide for Nursing Home Staff

For the past five years, Cobble Hill Health Center and Isabella Geriatric Center have been engaged in an ongoing collaboration to explore new ways to enhance person-centered care and to construct a model that uses performance improvement approaches to guide staff in addressing the needs and preferences of residents.

Under the guidance of an advisory board composed of representatives from several aspects of health and elder care, and coordinated by Ann Wyatt, both facilities explored ways to better integrate quality of care and quality of life for residents.

Through teamwork, communication and training, we were able to develop approaches to problem solving that include the perspectives of residents, families, direct care staff and managers.

This manual provides practical guidance to those nursing homes that may be exploring a similar journey toward enhancing person-centered care.

Getting Better All The Time was written by Project Coordinator, Ann Wyatt, MSW. A licensed nursing home administrator, Ms. Wyatt has extensive experience in the long-term care setting.    She is a Board Member of Isabella Geriatric Center, and of the Coalition for the Institutionalized Aged and Disabled (CIAD) and was a founding Board Member of the National Citizen’s Coalition for Nursing Home Reform and of the Village Nursing Home.

Getting Better All The Time is copyright protected and is available on both the Isabella Geriatric Center and Cobble Hill Health Center web sites.  This manual is made possible

through the financial support of the New York Community Trust, the United Hospital Fund and 1199 SEIU Training and Employment Funds.
