The selected candidate will be responsible for the planning, arrangement and coordination of services/resources to maximize the functional independence, and economic and social well being of assigned clients. The candidate will screen the appropriateness of service based on the thorough assessment the client(s) needs (and the needs of the family when appropriate). This position will work mainly with functionally or cognitively impaired individuals 60 years or older who are eligible for Department for The Aging funded meals on wheels, Expanded In-Home Services for the Elderly benefits and who are part of Social Adult Day Care Programs.

Requirements:

  • Bachelor’s or Master’s Degree in Social Work, Health, Human, or Education Services.
  • 1 year of case management experience with a geriatric population, and/or persons with a history of mental illness, homelessness, or chemical dependence.
  • Must have excellent writing and documentation skills.
  • Must be able to work with clients with multiple conditions/diagnoses.
  • Case management & supervisory experience preferred.
  • 4 years of full-time experience in social casework, or as a case aide in a human services agency also accepted.
  • Bilingual Spanish a plus, but not required.
