In order to qualify for Isabella’s Home Care Program you must meet the following criteria:
- Have a medical and/or mental health problem or disability and need periodic skilled nursing care
- Need assistance with personal care or be functionally limited
- Client, family and physician agree to participation
- Reside in Manhattan, The Bronx or Westchester
- Have Medicaid or be eligible; Medicare, Private Pay and Managed Care Insurance Plans also accepted
